SWSSC CONFERENCE REGISTRATION
POLICIES
-
REGISTRATION FORM: In an
effort to secure accurate information for each participant, we ask that you
submit a separate form for each attendee. Please make additional copies as
needed.

REGISTRATION DEADLINES:
Advance registration--In order to honor our attendance guarantees with the
hotel, completed form(s) with payment/purchase order must be postmarked no
later than March 30, 2006. Purchase order registrations may be faxed to
972-992-3134 no later than March 30, 2006.

LATE REGISTRATION:
Completed form(s) with payment/purchase order must be postmarked no later than
April 14, 2006. Purchase order registrations may be faxed to
972-992-3134 no later than 2:00 pm, April 25, 2006.

ON SITE REGISTRATION: On
site registration will be accepted. Please bring a completed
registration form with payment/purchase order information.

REFUNDS/CANCELLATIONS:
All refunds will be made after the Annual Conference. There will be a
$25.00 processing fee which will be deducted from each individual request.
Requests must be made in writing and postmarked no later than April 25, 2006.
Please include a copy of your cancelled check and registration form.
Registration fee(s) cannot be refunded for late cancellations and/or
"No-Shows". Purchase order registrations will remain payable in full for
late cancellations and/or "No-Shows".

SUBSTITUTIONS: In order
to keep our registration accurate, we ask that substitutions be requested in
writing and received either by mail or fax (972-992-3134) by 2:00 pm April 23,
2006 if you want a preprinted name tag, etc. Of course, we will always
accept substitutions on-site.

PAYMENT TERMS:
Payment/purchase order information must accompany completed registration
form(s). If available, please attach a copy of the purchase order.
We are unable to invoice purchase orders. Please use you registration
forms as an invoice. Make checks payable to SWSSC. Mail to the
address below.

GROUP DISCOUNT: In order
to receive the group discount for multiple attendees from a facility/agency,
please submit all registration forms together. Due to bookkeeping
requirements, late additions will be charged as a separate submission.

REGISTRATION
PROCESSING/CONFIRMATION: All registration submittals must abide by the
policies stated above to be processed. An incomplete submittal cannot be
processed. To keep the conference cost as low as possible, we will not
mail registration confirmations to attendees. If you wish to confirm
your registration, please call the number below.
